A4 PDF Migraine Tracker - Headache Planner

A year ago I was looking for a migraine diary. I needed a very simple design. My doctor prescribed medication and told me to track the number of days with headaches for one year. I tried keeping observations in an app that asks about pain severity, triggers, how long the headache lasted, associated symptoms, what medications I was taking and in what amounts, and so on, but I quickly got tired of these questions.

So I made a calendar like this for myself and just marked only the days when I had a headache. I coloured in completely if the headache was severe and marked with a dashed line if the headache was moderate. When you keep a calendar for a year, you can track the changes easily.
