This subcontractor qualification form is some what automated.  I designed it to be emailed to the sub contractors so they could click and fill out the form then email it back.  However, you can modify the form as you need then print, fax, or mail it with your subcontractor agreement package.

This is a nice clean form and saves me tons of time and works well with my sub trades.
